The Plot

Sarinya and Chai, two friends from Koh Phangan, arrive in Bali for what should have been a relaxing holiday. Their plans are quickly shattered when Chai is bitten by a monkey in the Sacred Monkey Forest, forcing a rushed trip to the overcrowded North Kuta Clinic. The building is chaotic: patients moan with fevered pain, hallways are littered with vomit and blood, and panic spreads faster than anyone can contain it. At first, the outbreak appears to be nothing more than a severe case of “Bali Belly,” a stomach flu sweeping the island – but it becomes clear that something far darker is unfolding.

Inside the clinic, the survivors meet Emma, a tough tourist armed with a baseball bat, and encounter both living and undead threats. Dr. Chen, a meticulous surgeon and virus researcher with a god complex, and his obedient nurse Wayan are revealed to have been conducting secret experiments on patients – experiments that may have triggered the outbreak by letting experimental virus-infected material enter the sewage system. Bodies rise as zombies, including the cursed real estate agent Eddie, turning the clinic into a nightmarish gauntlet. Sarinya, Chai, and Emma must fight, flee, and scavenge weapons just to survive.

The nightmare continues as the trio reaches Kuta Beach, a sun-bleached stretch of sand now crawling with infected beachgoers, drowned corpses, and mutated horrors. They must navigate collapsing structures, treacherous sand, and moral dilemmas as the outbreak spreads relentlessly.
The final act unfolds at the Denpasar Jetty, a collapsing harbor where waves of zombies threaten to trap the survivors. With limited resources and nowhere to run, the survivors must fight, improvise or sacrifice. You will play though three acts with lots of goose bump scenes. And with a touch of trash and splatter if you like! And for the DM you will find lots of tips and tricks how to create and even darker and nicely rotten atmosphere.

Player and GM roles

This 3 act adventure is designed for 2–4 players plus a GM. Sarinya, Chai, and Emma must be played by real players, as their skills and relationships drive the story and survival choices. All other characters – including Dr. Chen, Wayan, Eddie, Pamcake, Rico, and the infected – are controlled by the GM. The scenario balances tense, rule-based combat with narrative freedom, allowing the GM to shape encounters, manage horror pacing, and create emergent situations while keeping players on edge in a dark, deadly world. This adventure is fully compatible with Dungeons and Dragons 5e, using compatible mechanics, stat blocks, and combat rules for ease of play. Player characters, encounters, and challenges have been balanced for levels 3–5, offering tense, survival-focused gameplay. These levels are just a guideline – GMs are encouraged to adjust difficulty, modify encounters, or reskin threats to fit their party size, style, or preferred level range. The design keeps narrative and horror pacing flexible while. maintaining 5e compatibility.
